World Star-5 contains three Green Stars and a single Stamp to collect.

Hop to the collectible you need:
Green Star 1
Reach the top of the platform next to the warp block by using a Super Leaf to glide from the right, or by throwing a bomb at the four giant Question Blocks to form a set of stairs. After you enter the purple warp block, hit the giant Question Blocks to reach the Green Star within the time limit.
Green Star 2
After the Checkpoint Flag is a bridge made of brick blocks that leads to a single gray brick block. Throw a bomb at it to reveal the Green Star, but make sure to keep some of the bridge intact!
Green Star 3
At the very end of the second area is a Green Star at the top of the platform. Hit the giant Question Block to form stairs to the Green Star.
Stamp Location
At the end of the first area is a giant brick block wedged in the ground. Either throw a bomb or crash through in your Mega form to find the Stamp below.
