World Mushroom-5 contains three Green Stars to collect.

Hop to the collectible you need:
Green Star 1
Climb the second set of stairs with a baseball in hand. Throw it into the clear pipe to have it remove the spike. Enter the clear pipe and grab your Green Star!
Green Star 2
Propeller Block required! After the Checkpoint Flag, grab a Propeller Block and jump through the Green Star Ring. Collect all eight Green Coins to earn a Green Star.
Green Star 3
There is a warp block on the left set of clouds. Enter it to warp to the area with the last Green Star!
