I always have items off, I like it because it means that you actually have to fight instead of just grabbing a Hammer and being able to K.O everyone in a few seconds. I sometimes play with them on but that is rare :P

I hate having kids in this game, they don't age (Unless they learn the spell from Makin' Magic) and they always have to go to school which is a massive pain in the ass. They also are rather boring as well.

I have killed like 50 Sims, I have about 20 gravestones on my front lawn. It's an accomplishment in my world.
